The mobile phone field on the patient service form is used so the office can respond to the request. Entering a phone number by itself does not enroll a patient in SMS/MMS messages.

SMS/MMS consent is collected only when a patient separately checks the optional SMS/MMS consent checkbox in the patient service form. The checkbox is unchecked by default, is not required to submit the form, and is not required to schedule an appointment, purchase products, or receive treatment, service, or care. If the box is left unchecked, the patient is not enrolled in SMS/MMS messaging.

By checking the separate optional box, you provide your express written consent to receive recurring SMS/MMS text messages from Lakeview Eye Care at the mobile number you provided. Messages may include appointment scheduling, appointment confirmations, appointment reminders, patient follow-ups, office closures or schedule changes, contact lens reorder reminders, eyewear or contact lens order updates, and patient service questions. You may receive up to 8 SMS/MMS messages per month. Message and data rates may apply. Reply HELP for help or STOP to cancel SMS/MMS messages. Mobile opt-in information and consent will not be shared with third parties or affiliates for marketing or promotional purposes.
