These terms explain what patients can expect when they opt in to customer care SMS/text messages and MMS/picture messages from Lakeview Eye Care.

Lakeview Eye Care SMS and MMS Messaging Program
How You Opt In
Lakeview Eye Care collects SMS/MMS consent through one electronic method: the patient service form on the Lakeview Eye Care website at https://lakeview-eyecare.com/contact/.
On that form, patients may enter their name and mobile phone number and may check a separate optional SMS/MMS consent checkbox. The mobile phone field is used so the office can respond to the patient service request. Entering a phone number by itself does not enroll you in SMS/MMS messages.
The SMS/MMS checkbox is unchecked by default and is not required to submit the form, schedule an appointment, purchase products, or receive treatment, service, or care. If the checkbox is left unchecked, the patient service form can still be submitted and you will not be enrolled in SMS/MMS messaging.
By checking the optional SMS/MMS consent checkbox, you provide your express written consent to receive recurring SMS/MMS messages from Lakeview Eye Care at the mobile number you provided. Messages may include appointment scheduling, appointment confirmations, appointment reminders, patient follow-ups, office closures or schedule changes, contact lens reorder reminders, eyewear or contact lens order updates, and patient service questions.
You may receive up to 8 SMS/MMS messages per month. Message and data rates may apply. Reply HELP for help or STOP to cancel SMS/MMS messages. Mobile opt-in information and consent will not be shared with third parties or affiliates for marketing or promotional purposes.
Message Purpose
By opting in to receive SMS/MMS messages from Lakeview Eye Care, you agree to receive customer care messages related to your relationship with our practice. These messages may include:
- Appointment reminders.
- Appointment confirmations.
- Scheduling changes.
- Office updates.
- Contact lens reorder reminders.
- Eyewear or order updates.
- Limited follow-up communications related to your care or account.
- Responses to patient service questions.
MMS/picture messages may be used for limited service-related images or attachments related to office communication, eyewear orders, contact lens orders, or other patient service needs.
Our messaging program is intended for customer care and account-related communications. It should not be used for emergencies. If you need immediate care or have an urgent medical concern, please call the office directly.
Message Frequency
You may receive up to 8 SMS/MMS messages per month. Actual message frequency depends on appointments, orders, and service needs.
How to Stop SMS/MMS Messages
You can cancel SMS/MMS messages at any time. Just text STOP to our number. After you send STOP, we will send you a confirmation message letting you know you have been unsubscribed from SMS/MMS messages. After that, you will no longer receive SMS/MMS messages from us unless you submit the SMS/MMS consent form again.

Carrier and Pricing Notice
Carriers are not liable for delayed or undelivered messages. Message and data rates may apply for any messages sent to you from us and to us from you. For questions about your text plan, picture messaging, or data plan, please contact your wireless provider.
